Our dedicated, Sage Trained Employees, offer compassionate care and support, making residents and families valued. Daily programming encourages participation in activities that are meaningful and bring purpose to one’s life.
Transitions
Introducing our Transitions Memory Care Program, a steppingstone in the continuum of care for individuals with early to moderate stage dementia. This program is designed to provide a supportive environment for those who require reminders beyond what is offered in the Personal Care living community but are still too high functioning for the Sage Memory Care unit.
